Mar 5, 2011 · An ECG lead is a graphical representation of the heart’s electrical activity which is calculated by analysing data from several ECG electrodes. A 12-lead ECG records 12 leads , producing 12 separate graphs on a piece of ECG paper.
There are six basic leads discussed below and 6 precordial leads which will be discussed later. The basic leads consist of leads I, II and III and the augmented leads AVR, AVL and AVF. These are present on the basic 3-lead monitors and also on the 12-lead EKG machines.

The 12-lead ECG works by recording the heart’s electrical activity through ten electrodes placed across the patient’s body. Each lead captures the activity and outputs it as a waveform on a screen or piece of paper.
How to interpret the ECG: A systematic approach
Sinus rhythm (which is the normal rhythm) has the following characteristics: (1) heart rate 50–100 beats per minute; (2) P-wave precedes every QRS complex; (3) the P-wave is positive in lead II and (4) the PR interval is constant.
Understanding these essential elements of the normal 12-lead ECG should enable individuals to move on to a consideration of the abnormal ECG, key aspects of which are covered in Parts 2 and 3 of this three-part series.

There are 6 steps to interpreting a 12 lead ECG. Since step 1 is to do the basic ECG interpretation, that module is not included. On any 12 lead ECG, the first thing you do after the basic interpretation is the determine if the patient has an axis deviation. This is determined using leads I and AVF.
