Hey. About a year ago, this time, I drove the N4 between Nelspruit and Malelane gate and had no issues. The N4 for is normally safe, just very busy.

You're supposed to keep the windows of your car closed all the time, however, if you are hiring the local transport with open windows, it might help.

You will still need to pay the gate fees when entering the park, however the conservation fees will probably be included in your lodge booking (if you are staying in one of the private lodges). I recommend you just check with the lodge beforehand. So yes, conservation fees still apply and goes for a good cause.
